The usage of the below forms will vary depending on the intensity of your higher risk activity. A day trip participating in archery will look different than a multi-day expedition trip to a remote location. Please use your discretion in reviewing the below forms to see what is best fit for the activity you are planning. Many of these forms are available to help best prepare you for planning your trip and taking into consideration aspects of trip planning you may otherwise not have thought of.
Consider touching base with your principal to help you with this process on what they feel is best suited for your specific activity.

Higher Risk Activities Forms & Documents
- Review school procedures in AP-260 Overnight_Travel_Within_British_Columbia before submitting to school board office.
- Principals approve requests to and submit Form 260-05-Proposal for Higher Risk Activities to the Superintendent or designate (Associate Superintendent) via email to jay.dixon@sd71.bc.caand cc Marlene Leach Marlene.Leach@sd71.bc.ca
- Parents complete Form-260-06-Parent-Guardian-Consent-for-Higher-Risk-Activities
- All supervisors and volunteers must complete code of conduct- AP 490-Volunteer Code of Conduct – https://forms.office.com/r/P90Ztb7LeB
- All supervisors and volunteers must complete conduct and criminal record checks AP 490- Criminal Record Check Volunteers
- All volunteer drivers must have Form 260-02-Volunteer Driver Application
- Report any injuries during the trip or incidents promptly to Associate Superintendent.
- Only district approved transportation charter / bus companies may be used.
